It doesn’t mean you’ve been overlooked, it means your providers have ruled out all standard causes and may need to dig deeper. The diagnosis opens the door to a more personalized and observant approach.

Some common causes for unexplained infertility include:

Previous surgeries, environmental factors, and even mild endometriosis may influence conception in ways that routine testing can’t fully catch. Age can also shift the odds, especially after 35. Egg quantity and quality can quietly decline over time, even if regular cycles and hormone levels remain steady.

What Happens During Diagnosis?

Even though unexplained infertility is diagnosed after testing, your provider may want to take a second look at your results or recommend a few more targeted evaluations. Every lab value, scan, and past experience helps paint a clearer picture.

We may check ovulation patterns, review hormone levels across multiple cycles, perform advanced sperm function testing, or look for minor uterine or tubal issues with updated imaging. These steps help ensure we’re moving forward with as much information as possible.

You’ll also discuss lifestyle factors, such as exercise, sleep, diet, and exposure to heat or chemicals. While these may seem minor, they can have a quiet impact over time.

What Are the Treatment Options?

Once unexplained infertility has been diagnosed, your provider may recommend empirical therapy, which is a treatment based on collective experience and proven outcomes, even when a specific diagnosis isn’t available. At our fertility center, we typically explore this through three levels of support:

Level One: Timed Intercourse with Medication

  • Oral medication and/or low‑dose injectable medications encourage ovarian follicle growth.
  • In‑office monitoring and lab work track follicle development and hormone levels.
  • Intercourse is timed precisely around ovulation to maximize the chance of egg and sperm meeting naturally.

Level Two: Intrauterine Insemination (IUI) with Medication

  • Oral and injectable medications stimulate multiple follicle development.
  • In‑office monitoring and labs guide the timing of the procedure.
  • A prepared sperm sample is placed directly into the uterus at the optimal moment, raising the likelihood of fertilization.

Level Three: In Vitro Fertilization (IVF)

  • Injectable medications produce multiple mature eggs.
  • Eggs and sperm unite in the laboratory, enabling direct assessment of fertilization and embryo growth.
  • Embryo selection and transfer occur under precise conditions, offering the most controlled path to pregnancy.

Your treatment plan will depend on your goals, medical history, and how your body responds to each step. Some people may achieve pregnancy through the first approach, while others may need to explore IVF for better clarity and results.
